Job description

1. Use clinical pharmacy prescription approval module to review and authorize medication orders digitally. 2. Detect and mitigate medication errors proactively. 3. Perform IP and OP prescription audits, initiate corrective and preventive actions (CAPA). 4. Develop, prepare, and revise the hospital drug formulary. 5. Audit on safe usage of narcotics and psychotropic drugs. 6. Participate in daily ward rounds with clinical teams. 7. Ensure adherence to hospital medication management policies. 8. Coordinate for Antimicrobial Stewardship Program (AMSP). 9. Serve as member of Hospital Infection Control,Pharmacy and Therapeutics Committee, Ethics, and Quality Assurance committees. 10. Attend meetings and present data on medication management indicators. 11. Conduct pharmacovigilance including Adverse Drug Reaction (ADR) monitoring and Reporting with CAPA. 12. Provide staff training on medication management. (MOM). 13. Evaluate medication orders for appropriateness of dose, route, frequency, and duration throughout hospital stay. 14. Ensure rational prescription of drugs to maximize efficacy and minimize adverse effects. 15. Coordinate with doctors to adjust medication doses for renal and hepatic impairment. 16. Deliver drug information services, answering queries from healthcare professionals. 17. Monitor surgical antibiotic prophylaxis and ensure compliance with protocols. 18.
