Plan Navigation: Help clients interpret funding, budgets, and rules within their care or government plans (such as the NDIS).Service Connection: Research and link clients to appropriate local, community, and mainstream providers.Capacity Building: Train and empower clients to manage service agreements and direct their own supports.Monitoring & Reporting: Track service delivery, check in regularly with clients, and document goal achievements or plan adjustments.Crisis Resolution: Solve practical challenges when service gaps or unexpected issues arise. Pay: ₹18,000.00 - ₹21,000.00 per month Work Location: In person
