Nursing Registration: State Nursing Council Registration is mandatory Role Overview: The Liver Transplant (LTP) Nurse provides specialized, evidence-based nursing care to liver transplant candidates, living donors, and recipients throughout the transplant journey, from pre-transplant evaluation to post-transplant follow-up. The role ensures patient safety, multidisciplinary coordination, patient education, regulatory compliance, and optimal clinical outcomes. Key Roles and Responsibilities: • Conduct comprehensive nursing assessments of liver transplant candidates, donors, and recipients. • Coordinate pre-transplant evaluation, investigations, and patient readiness for transplantation. • Assist in transplant listing processes and ensure completion of required documentation. • Prepare patients for liver transplant surgery by completing pre-operative assessments and education. • Coordinate with transplant surgeons, hepatologists, anesthesiologists, ICU, OT, laboratory, blood bank, radiology, and other multidisciplinary teams. • Provide specialized nursing care during the immediate post-operative period. • Monitor vital signs, neurological status, hemodynamic parameters, liver function, fluid balance, urine output, drains, and surgical wounds. • Assess for early signs of graft dysfunction, rejection, bleeding, infection, biliary complications, vascular complications, electrolyte imbalance, and hepatic encephalopathy. • Administer immunosuppressive therapy, blood products, antibiotics, analgesics, and other prescribed medications safely. • Monitor laboratory investigations including liver function tests (LFTs), coagulation profile, CBC, electrolytes, renal function, tacrolimus/cyclosporine levels, ABGs, and other transplant-related parameters. • Maintain strict infection prevention and aseptic practices for immunocompromised patients. • Educate patients and caregivers regarding immunosuppressive therapy, medication adherence, infection prevention, nutrition, hygiene, activity, follow-up care, and recognition of warning signs. • Provide psychological support and counselling to patients and families throughout the transplant process. • Coordinate multidisciplinary discharge planning and continuity of care. • Arrange follow-up appointments, investigations, and long-term transplant clinic reviews. • Maintain accurate, timely, and complete documentation in the Electronic Health Record (EHR). • Ensure compliance with hospital policies, liver transplant protocols, NABH standards, and regulatory requirements. • Participate in quality improvement initiatives, clinical audits, infection control, and patient safety programs. • Recognize and respond promptly to clinical deterioration and transplant emergencies. • Participate in patient education, staff training, competency assessments, and continuing nursing education. • Maintain patient confidentiality and uphold ethical practices in accordance with the Transplantation of Human Organs and Tissues Act (THOTA) and institutional policies. Other Educational Function • Participates in clinical teaching both planned and incidental. • Helps in the orientation of new staff. • Participates in staff education program. • Guides student Nurses. Liver Transplant Management • Coordinate pre-transplant evaluation, patient preparation, and readiness for liver transplantation. • Verify completion of pre-operative investigations, informed consent, blood availability, and transplant documentation. • Prepare patients for liver transplant surgery in accordance with institutional protocols. • Provide comprehensive post-operative nursing care in the ICU and ward settings. • Monitor vital signs, neurological status, hemodynamic parameters, urine output, drain output, fluid balance, and pain levels. • Assess liver graft function by monitoring liver function tests (LFTs), coagulation profile, renal function, electrolytes, blood glucose, and immunosuppressant drug levels. • Identify and promptly report signs of graft dysfunction, acute rejection, infection, hemorrhage, biliary leak, vascular complications, hepatic encephalopathy, and other post-transplant complications. • Administer immunosuppressive therapy, antibiotics, blood products, intravenous fluids, and other prescribed medications safely while monitoring for adverse effects. • Maintain strict aseptic techniques and implement infection prevention measures for immunocompromised transplant recipients. • Coordinate multidisciplinary care with hepatologists, transplant surgeons, intensivists, anesthesiologists, pharmacists, dietitians, physiotherapists, laboratory, and radiology teams. • Educate patients and caregivers on lifelong immunosuppressive therapy, medication adherence, nutrition, infection prevention, activity restrictions, warning signs, and scheduled follow-up care. • Plan safe discharge, ensure continuity of care, and maintain accurate documentation in accordance with transplant protocols, hospital policies, and regulatory standards. • Provide pain management and wound care as per protocol. • Maintain strict infection prevention and isolation precautions. • Educate patients and caregivers regarding medication adherence, infection prevention, diet, hydration, lifestyle modifications, and follow-up care. • Coordinate multidisciplinary care with Hepato-Pancreato-Biliary (HPB) and Liver transplant surgeo
