**Job Title** : **Phlebotomist (Max@Home) Max Healthcare** **Location :** Max (Kalinga Hospital Square,) Maitri Vihar, NALCO Nagar, Chandrasekharpur, Bhubaneswar, Odisha **JOB PURPOSE** MAX@Home A division of max healthcare institute Ltd. Among the all services MAX@Home also provide home sample collection service and to carry out blood sample/phlebotomy from patient is of various samples and reach the patient/clinician at the required time. **Role & responsibilities** - To effectively interface with patients, attendants, nursing staff and service users. - To demonstrate skills in handling blood & sample collection. - To do collect blood sample from patients door Step - To strictly follow and implement the laboratory SOPs and Quality assurance programs. - To implement infection control policy & proper waste disposal. - To comply with the service quality Process, environmental & occupational issues & policies of the respective area. - To comply with patient safety policy. - To ensure safe utilization of equipment and proper waste disposal system - To ensure daily patient details should be updated in handover and daily payment should be submitted to Finance team/FO team **Home Sample Collection** - Home Collection - Reach on time at patient door step for sample collection - Collect blood sample from Patients - Daily Document Use - TRF/Bill book Mandatory fill up all blank on bill book page at patient home on real time (Patient name/Age/Gender/package or test/email ID/payment/payment mode (cash/card/pre-billed)/patient signature etc - Tent card Should be used on real time at patient home on all sample collection - Vein Finder - Should be used on all patient sample collection - Daily use shoes cover, gloves and mask - Daily use butter fly needle, vacutainer, syringe for sample drawn - Fulfillment beg along material, vein finder, temperature control sample collection Kit pouch (vials, Banded, swab, gloves, needle) tourniquet, vacutainer holder, marker **Duty & Document Compliance** - Reach at patient home location on time (Punctuality) - Sample submission at hospital on time within 2-3 **Trainings** - Ensure participate in all technical training, - Ensure Process according to Drill Video, - Ensure participate in 10 behavior Expected training **Technology Platform** - Mobile application should be installed and login in phlebotomist mobile phone - Real time uses of Application at patient location and no deviation from the process - Full compliance of on Pathology Application (100% app adoption ) **Qualifications** 10+2 with Diploma (MLT)/ BSC (MLT) **Experience** Preferred 1-5 years of home sample collection experience Interested candidates may send their profiles deen.dayal@max@healthcare.com **Contact :** Mohd Ruman **# 8929286241** Regards
